Bao Viet has some responsibilities for the government. For example, Bao Viet must pay tax for their income, in other words, the company must follow all the regulations and laws which the government have issued. Besides, they also make benefits for the local communities such as reducing the ratio of unemployed people in the area. Secondly, we have the ethics and business. Obeying the law is that the company must follow the laws and regulations to do the business. Furthermore, the company has to respect the rights of competitors. The firm has not to criticize or attack the rights of competitors or do everything which can harm the competitors. Business ethics also includes that the organization should protect the employees and consumer from danger of the business. In addition, the company also must not do some illegal activities such as bribery or gifts. The next one is the management responsibilities. The company should have good relationship with all their stakeholders. Now, Bao Viet has their IPO, so they will have more shareholders. In addition, the business has the responsibility to maximize the profits for shareholders. As a result, they will continue to invest in the company and the business will make more benefits. Second one is the employees who keeps the organization in operational existence. They contribute for the company by joining in the production directly so their position is also very important and they should be received a suitable treatment in terms of wages or salary, bonuses, etc. Moreover, the firm also should give the employees a coherent career and a training structure to make them become better and work more efficiently. Meanwhile, the company needs to make a good environment for workforce’s lifestyle and make a good condition for them to express all their talent. Next, I will talk about the responsibility for customers. For Bao Viet, the enterprise has to listen to every feedbacks of the customers, develop the quality of services and keep contacting with them to provide enough information which the customers want to know. For the suppliers, the firm must not delay payment or use the power unscrupulously. In addition, having a long-term relationship or giving new supplier the chance to win new business also makes benefits for both sides. The business also have some responsibilities for the competitors such as the laws of competition about fair trading, monopolies or mergers, etc. The last one is responsibilities for the Community. Each area has their own social and ethical values, so that the firm has to respect these values. Moreover, when the Community needs more capital to hold up an event or charity, the business also should sponsor for them. This way will also make benefits for the company because they have a chance to spread their brand. In addition, the last one is that the enterprise has to respond to complaints for local residents. In this part, I will give some strategies which Bao Viet has followed to meet the objectives of stakeholders. She believes that mister Watts, Matilda’s teacher and the only white man left on the island, has a bigger influence on Matilda’s life. This is the reason why she steals the novel, Great Expectations written by the novelist Charles Dickens, which Mr. Watts has been discussing and reading with his class. This is a very dangerous act, since the Redskins, who took the power over the Solomon islands , want to see the novel as a prove of the non-existence of mister Pip. In the play Hamlet Ophelia’s brother Laertes seeks for more power and respect from Claudius especially after his father was killed by Hamlet. By wanting this he becomes a danger to others. He wants revenge on Hamlet and plans, together with Claudius on killing him. By this he becomes a danger to young Hamlet. Political power is a reason for people to become a danger to others. In the beginning of the play Hamlet young Fortinbras plans on attacking Norway, because he wants more political power. In this way he becomes a danger to Norway, but also to his people because this could start a war. In the novel Mister Pip the redskins have the political power over the Solomon Islands. They formed a danger to the inhabitants of this islands. When they took over power they started a war between the redskins and the rebels, which caused a lot of deaths. People who want to take over power are very dangerous to others. In the play Hamlet, Claudius, old Hamlet’s brother, kills old Hamlet because he wants the power over the country. Furthermore he plans on killing young Hamlet, since young Hamlet is against Claudius haveng this power. Claudius becomes a danger to others by trying to kill them because he wants power. In the novel Mister Pip the rebels, who are against the red skins who took over power, want to take their power back. They are a danger to others since they get drunk on jungle-juice every night to handle the pressure and they rape women on the island. Their needs and will to take the power over the Solomon islands back are a danger to the people living on the islands. People who have a thirst for power are dangerous to others. When they want to take over power or become more powerful this can cause them to act in a way that is dangerous for others. This is clearlt shown in the two works studied, the novel Mister Pip and the play Hamlet.

Identity and Postmodernism | Essay Critically assess the contention that identities are, plural, unstable, situationally enacted, and sites of contestation. The stability or otherwise of identity has become a major battleground for sociological theorists in recent times. The infamous ‘postmodern’ turn has rendered identity a deeply problematic phenomenon. In this paper I will investigate the claim that identities are unstable sites of contestation. I will do this by examining the dissolution of identity within postmodern theory before examining both the negative and more importantly, the positive consequences of this.[1] This will enable a deeper understanding of precisely what is meant by this fluid notion of identity, and where possible criticisms and inconsistencies can be located within this theory. The debate over the stability of identity is one that is inseparably linked to postmodernism. This diverse group of theories centre around, in Lyotard’s (1984:xxiv) famous phrase, ‘incredulity toward meta-narratives.’[2] Postmodernists maintain that the project of modernity has failed, and that no single source or body of knowledge can legitimise itself as a universal measure of value or identity. This obviously has some profound effects on the ways in which we would normally think about the world. Postmodernism no longer allows us to theorise society into homogenous identities which can then be totalised in a grand-theory or meta-narrative. This is also the case when it comes to the identification of the self. Rather than the self maintaining a stable core of identity, from a postmodern perspective identity is fluid and is dependant upon where the self is historically and culturally situated. As Luntley (1985:185) notes, this conception of the self threatens the very possibility of self-identity: The loss of self-identity is threatened because if we situated the self in real historical circumstances, we would situate it in things that are contingent and constantly changing. Therefore, the self would also be constantly changing. It would be in flux and would have no continuing identity. Once the very identity of the self comes under threat, then so does the possibility of any coherency in social theorising. A postmodern society is one in which the identities of the social actors are undergoing constant transformation. Identity then becomes open to contestation as there is no longer any ultimate referent (truth, science, God etc.) to provide universal legitimation. In Lyotard’s terms, the impossibility of a grand or meta-narrative leads to the social being constructed of small narratives, none of which are necessarily more valid than another. Any theory that aims at totalising society should only be seen as one constructed from a particular perspective (e.g. one that still remains in the logic of modernity), rather than a totalising theory as such. Whilst postmodernism can be viewed as liberating and opening up seemingly limitless opportunities for re-theorising society, it does at the same time impose new problems. Firstly, there seems to be an inconsistency in the postmodernist stance, as it could be argued that the theory of the dissolution of meta-narratives is a type of meta-narrative itself. This criticism can also be applied to the postmodernist take on identity, for in arguing that identity is ultimately unstable and fluid postmodernists inadvertently provide a certain rigid structure in which identity operates (i.e. that all identity must be unstable). So whilst postmodernism is liberating on the one hand, on the other it sets limits to the very possibility of any meaningful social theory or practice. This is exemplified in the disparity between postmodern theorists, some of which view postmodernism as opening up huge opportunities for getting rid of authoritarian grand theories, others view it as essentially debilitating as the only thing that can prevail in postmodern societies is a sense of meaningless flux. Within this disagreement the postmodern analysis of identity remains reasonable intact, both sides of the argument larg ely accept that identity is fluid and unstable. By analysing this disagreement we can therefore obtain a better understanding of the various aspects of fluid identity. Jean Baudrillard (1990:160-164) for example, argues that the dissolution of identity is a process that started in the nineteenth century and was exacerbated in the twentieth. In the postmodern era, historical processes have undermined the stability of identity, so that it becomes impossible to meaningfully theorise about social identity. Rigid identity and meaning are destroyed due to the rise of global capitalism and the demise of the referents from modernity (truth, purpose, meaning and so on). ‘Gone are the referentials of production, signification, affect, substance, history, and the whole equation of â€Å"real† contents’ (Baudrillard 1988:125). Identity now becomes a radically fluid and empty vessel, which becomes temporarily filled with content that has no foundation or ultimate meaning. Whilst for Baudrillard this cannot be thought of as a particularly positive or negative phenomenon, as ‘good’ or ‘bad’ no longer have any real mea ning in postmodernity, it does render theoretical and political action largely impotent.[3] This is why in postmodernism we are presented with numerous texts heralding the end of theory, history, meaning and so on.[4] The dissolution of identity means for many postmodernists that theory and meaningful political action are no longer possible: The end of history is, alas, also the end of the dustbins of history. There are no longer any dustbins even for disposing of old ideologies, old regimes, old values †¦ Conclusion: if there are no more dustbins of history, this is because History itself has become a dustbin. It has become its own dustbin. Just as the planet itself is becoming its own dustbin. (Baudrillard 1994b:26) The negative aspects of the lack of fixity and grounded meaning in identity are thus very evident. Laclau and Mouffe on the other hand, in Hegemony and Socialist Strategy, positively embrace the fluidity and instability of identity. Indeed, they argue that the impossibility of the closure of identity is what makes the social possible (1985:112). Society as such is therefore an impossible object for Laclau and Mouffe, as the field of identities is never fixed, but the continuing attempt to do this renders the possibility of the social. Society resists closure and remains eternally negotiable as the meanings produced to bind the social together are only temporarily fixed at nodal points by articulation (1985:11). Articulation is where social relations and identities are modified. Many differing types of articulations (political, cultural, scientific an so on) are capable of doing this, but the important thing for Laclau and Mouffe is that no one particular articulation totalises and re stricts the ability for other articulations to operate freely. Laclau and Mouffe (1985:13) argue that their concept of hegemony recognises the plurality of struggles and attempts to engage with it: The concept of ‘hegemony’ will emerge precisely in a context dominated by the experience of fragmentation and by the indeterminacy of the articulations between different struggles and subject positions. Hegemony for Laclau and Mouffe refers to the ‘battleground’ of identity. As the identity of the social is fluid and open to negotiation, different types of social articulations and struggles will attempt to hegemonise society to gain recognition. While this attempt at hegemony in itself is not a negative practice for Laclau and Mouffe, successfully achieved hegemony is. It is therefore imperative that a strong egalitarian and democratic framework is in operation for this site of social hegemony. The advent of democracy is therefore a pivotal moment in social history. Here Laclau and Mouffe (1985:186-187) concur with Claude Lefort’s analyses of the ‘democratic revolution’. Society prior to democracy was thought of as a unified body with power being embodied through that of a sovereign monarch, who was the representative of a god or gods. After the democratic revolution, power becomes an empty space without reference to a transcendental guarantor or a r epresentation of substantial social unity. A split occurs between the instances of power, knowledge, and the foundations of law which are no longer absolute. Without these foundations, no law can be fixed and everything is open to questioning. Society cannot be apprehended or controlled, the people become sovereign but their identity can never be totally given. But once we are in a democratic society, we are in danger of totalitarianism. This is because a purely social power can emerge after democracy has destroyed extra-social powers, which presents its power as total and extracts from itself alone the principles of law and knowledge. As there are no longer any foundations or a centre to political power, it becomes necessary to bind together political spaces through hegemonic articulations. But these articulations will always remain partial, as they have no ultimate foundation. Any attempt to deny the radically open nature of the social will lead to totalitarianism, be it a politic s of the ‘left’ according to which every antagonism can be eliminated and society rendered transparent, or a fascist authoritarian fixing of the social into a rigid hierarchical state system. The democratic logic of equivalence can therefore be hegemonised into totalitarianism. The radical openness of identity is therefore impinged with the danger of totalitarianism for Laclau and Mouffe.[5] To avoid this, the diverse and fluid nature of identity should be embraced within an egalitarian and democratic framework, so no particular articulation may hegemonise social identity. This is difficult however as the ultimate lack of closure for identity leads to a necessarily antagonistic network of social relations. Antagonism is caused when a discursive form of one type of identity interrupts another’s discursive frame (1985:154). The inability of a particular identity to successfully assimilate the articulations of another leads to an internal antagonism that becomes the catalyst for a further modification of itself. Hence there is no stable core to any particular identity, identity is always shifting and changing. But this is also how a democratic framework can be constructed. As all identity is open, then democratic and egalitarian ideals can permeate diff erent articulations to avoid totalitarianism: [I]t is only from the moment when the democratic discourse becomes available to articulate the different forms of resistance to subordination that the conditions will exist to make possible the struggle against different types of inequality. (1985:154-155) The openness of identity, once incorporated into a democratic framework, is therefore a positive and progressive phenomenon for Laclau and Mouffe. The impossibility of totalising society is embraced as an opportunity for new fields of thought to be created, free from the tyranny of authoritarianism. We can therefore see a great disparity between Baudrillard’s and Laclau and Mouffe’s notions of the openness of identity. Both perspectives fully accept the lack of stability in identity, yet for Baudrillard this leads to sociological and political impotence, whereas for Laclau and Mouffe this is seen as an opportunity for sociological and political creativity and action. For many theorists however, the apparent differences or similarities between various postmodern theories of unstable identity are merely superficial.[6] They claim that there are deeper problems and inconsistencies within this notion of identity itself. Zizek (2000:106-107), for example, claims that whilst Laclau and Mouffe are vehemently opposed to all forms of essentialism, and seek to affirm the radical contingency of the political and irreducibility of the social, they nonetheless have to rely on a formal existential a priori, such as ‘the logic of hegemony’. In other words, one of the main problems with this type of discourse is that in maintaining that identity and the social is radically open, it has to rely on a certain formal logic. Laclau and Mouffe have to rely on a ‘logic of hegemony’ as the natural state of identity formation and articulation, as they deny that the fluidity of identity is a historical phenomenon: Only in contemporary societies is there a generalisation of the hegemonic form of politics, but for this reason we can interrogate the past, and find there inchoate forms of the same processes that are fully visible; and, when they did not occur, understand why things were different. (Laclau 2000:200) This proposes that all social identity was always-already the result of hegemonic struggles, whilst it is only in our ‘postmodern’ world that we can recognise this. So while the maintaining of the openness of identity is a form of anti-essentialism, it is nonetheless only operable within a rigid essentialist framework. Zizek criticises this approach for its lack of historical analysis. For Zizek (2000:95) it is the process of contemporary global capitalism that has created the conditions for the demise of essentialist politics, and has led us to the ‘recognition’ of the irreducible plurality of identities.[7] Zizek argues that Laclau and other proponents of this postmodern notion of identity do not analyse the logic that makes this possible, and therefore do not engage with any theoretical confrontation with it. In fact Zizek (1993:216) and other notable theorists argue that postmodern theories of identity are merely a product of capitalism and late modernit y:[8] Far from containing any kind of subversive potentials, the dispersed, plural constructed subject hailed by postmodern theory simply describes the form of subjectivity that corresponds to late capitalism.’ Rather than postmodern identity being a liberating and revolutionary new way of rethinking the social, from this perspective it is merely a reaction of late modernity which fails to seriously engage with the major problematic of our time. It is in this sense that Hardt and Negri (2000:138) argue that ‘the postmodernist and postcolonialist strategies that appear to be liberatory would not challenge but in fact coincide with and even unwittingly reinforce the new strategies of rule.’ Postmodern notions of the fluidity of identity bring us to a political and theoretical impasse.[9] But it could be argued that this is only the case if we accept postmodernism itself as a type of totalising theory. The notion of the fluidity of identity is useful and does open up new avenues of theorising and politicising. But as Zizek and others argue, the social and historical processes that have lead up to this should play a greater role in understanding modern or postmodern identity. Some postmodernists such as Baudrillard accept these historical processes, but insist that they are irreversible under a banner of the end of history. Others such as Laclau and Mouffe insist on the positive aspects of the instability of identity, and indeed even insist that it is unavoidable. But what both these positions share is the unavoidability of groundless identity, and the ultimate impossibility of creating positive content for identity. Laclau and Mouffe may argue that positive identity is possible, within a democratic framework. But the problem of failure remains unavoidable; all identity is either a failed attempt at hegemonising the social, or if successful then it is necessarily totalitarian as it denies the radical openness of identity as such. Even in this positive use of fluid identity, negativity is still very much inscribed into its operation. The lack of fixity in identity does indeed seem to correlate with modern or postmodern subjectivity, as Zizek argues above, but claims that make this a universal and necessary phenomenon are fraught with difficulties. Another definition can be used here, that ambulatory surgery is the performance of planned surgical procedure with the patient being discharged on the same day [2]. The ambulatory surgery first found in 1909 by James Nicoll, a scottish surgeon, it was called by day case surgery. In 1912 Ralph Walter in the USA adopted this surgery type in the USA. It was unpopular until the 1960s and 1970s when the traditional surgeries became a bottleneck for most of the USAs Hospitals, where keeping the patient on holding list and admitted them in the hospital became more expensive, in addition the availability of beds decreased. Walter Reed introduced the ambulatory surgery to USAs hospital, since then patient manages improved significantly and rapidly with ensuring the patients fitness after discharge [2]. Ambulatory surgery form about 90% of all surgery performed nowadays in Canada and USA [6]. The day surgery can achieve high level of quality, cost effective and safe which lead to high level of patient satisfaction [6]. University of California at Los Angeles developed a hospital based on ambulatory surgery unit in 1962, then other units in the USA were op ened in 1966 At Gorge Washington university, until big number of ambulatory surgery is opened now in the USA and Canada [7]. Several associations created to developed a strategies and plans to adopt and improve the ambulatory surgery, one of these association is the Federated Ambulatory Surgery Association (FASA), this association founded in the USA since 1974, another 12 national association formed and become member of the International Association for Ambulatory Surgery (IAAS) [8] The advantages from ambulatory surgery system are varied in type, some of these advantages related to patient and their family and some related to the hospitals and the healthcare system as whole. Those advantages for the patient that they will receive more attention from the healthcare team, because the ambulatory surgery designed to serve that patient [9]. The ambulatory patient will return home after receiving the treatment, so it is better to well manage the day surgery units and provide the patient with treatment which allowed them to continue recovering at their family home environment. Small mistakes that could happen for inpatient will not occur to the ambulatory surgery patient, like missing drug or shot or give different medicine for patient, because in the ambulatory surgery patient is always having everything in plan and no mistakes there [10]. Day surgery is better for children than inpatient surgery where the children will not be separated from their family for long time. The children will be less stressful and feel more comfortable because they can join back their family after that surgery finish [11]. In the European Charter of Childrens Rights states that children should be admitted to hospital only if the care they require cannot be equally well provided at home or on a day basis [12]. Other main important advantage is that scheduling for ambulatory surgery is much easier and less complicated for registration. Furthermore the day surgery will not be cancelled because an emergency admission likes what happen to inpatient surgery. The recovery period for ambulatory surgery patient is faster than inpatient, this allowing to patient to return to the normal life sooner [13]. For the hospital the benefit is more in way of management way where the efficient for providing surgery and the flow is less risky where everything is scheduled and prepared without facing cancelation, this offer more utilization for the facility and resources and give the patient more booking choice [14]. All healthcare organizations are having problem with budgets and limit funds and the patient demand also increased at the same time, the new treatment way for making same day surgery operation is cutting down the cost and reduce the demand for saving beds and resources for inpatient who waiting their surgery to place; while the ambulatory surgery patient receiving high quality and effective treatment those inpatient having shortage sometimes because limitation and long scheduling [8]. The first found of ambulatory surgery was between 1864 and 1921 by James Nicoll while his working in Sick Childrens Hospital and Dispensary in Glasgow, Scotland. Most of his operation was on special condition like phimosis, mastoid disease, cleft palate and spina bifida that all were done in daily basis, where the children received treatment within the same day. James Nicoll reported that 8988 paediatric cases were success in 1909 [2]. Since that time to 1948 nothing changed and the ambulatory surgery did not change in the way to adopting it, in 1984 British Medical Journal reported that surgeon allows a patient to leave hospital within 14 days of an intestinal operation [15]. The idea was adopt by different surgery around the world but none designed a unit for ambulatory surgery, until 1962 when a hospital build a unit at the University of California at Los Angeles, USA based on Nicolls concept [16]. Another unit opened within USA at George Washington University in 1966, after that in 1968 another unit designed in Providence, Rhode Island [17]. Reed and Ford both opened their Surgicenter in Phoenix, Arizona in 1969; it was the first similar idea to what Nicolls units was [7]. In period between 1970s and 1980s a large number of day units opened in Canada, the USA, the UK and Australia and started reporting in medical journals the benefits of having day surgery and the procedures and the way of adopting them. The key elements of those publications were the quality, cost effectiveness and safety of these ambulatory surgeries, and they published in different places like Orkand Corporation in the USA, the Royal College of Surgeons of England and the Audit Commission in the UK and the Royal Australasian College of Surgeons of Australia [18-22]. Different association started to form in order to organize the work of ambulatory surgery units and deliver the new technique and methods to other units and maximize the benefit of having ambulatory unit attached to the hospital and separate surgery unit. Their Main goal is to promote quality standard expansion, education and research in the field of ambulatory surgery centers. The first foundation was the Society for the Advancement of Freestanding Ambulatory Surgery Centers (FASC) which now known as Federated Ambulatory Surgery Association (FASA), that association founded in the USA in 1974. Another 12 American association decided to form in 1995, and all become members of the International Association of Ambulatory Surgery Centers (IAAS) [8]. (c). How do ambulatory surgery centers influence healthcare delivery? In healthcare delivery system it is important to provide the patient with the appropriate treatment according to his/her case with high quality and effectively. The inpatient facing some difficulties in booking a date and time for his/her surgery and sometime they need to stay in the hospital for recovery, this amount of load on the hospital may lead to some mistake in healthcare delivery process where mostly happened in medication is. This scenario cost the hospital a lot of resources and dollars in order to fixing them, this misleading in manage the inpatient, most likely, not found in ambulatory surgery patient. The amount of accuracy the treatment deliver is high, moreover the quality and efficiency also acceptable. According to the nurse who their ages averages between 41 year in the UK and 45 in the USA, the ambulatory surgery unit is more attractive to them, where they need more time to spent with their family and the needs for weekend and overnight will decreasing. With fixed schedules for surgery in ambulatory surgery unit it gives the opportunity for nursing to work overtime with more efficiency and effectiveness. The efficiency of nurses in surgery is important where it cannot reach the highest value in the normal inpatient surgery which may be taken over night or weekend. [8]. According to the cost of inpatient surgery compared to the ambulatory surgery it can be significantly different, usually for inpatient surgery reserving bed for patient is important without knowing the period of time that patient need for recovery and weekend and night staff is necessary, these two facts added cost to the hospital where if it is ambulatory surgery the need for hospitalization time will be reduced and night/weekend staff is not required. In the ambulatory surgery units the time and staff are used more intensively and effectively. The average unit cost savings of between 10% and 70% is documented in case of having ambulatory surgery rather than inpatient surgery [8, 18]. The improvement of patient throughput is one of the advantages of having day surgery unit [14], another benefit is to provide an enhanced patient service in a cost effective manner [8]. For Children most cases are performed in ambulatory surgery unit, it is rule, unless it is necessary to consider them as inpatient. This will reduce the stress on the patient and their parents also elder people will recover fast in their home after having one day treatment in the hospital. The way of deliver the treatment in that way will improve the patient satisfaction and improve the efficiency of the healthcare [12]. If we consider the study of patient turnaround time and length of stay, these two measurements are good indicator of how effective is the healthcare treatment was, less length of stay is the case where we have ambulatory surgery unit working and the aim is to reduce the time that spend between starting the triage until the patient discharge. The quality of treatment is also a case where we need to consider less variability of process and high patient satisfaction, less autonomy is the key point in the ambulatory surgery units where everything is schedule and designed in way to have best treatment without mistakes. The load inside the hospital to reserve beds and prepare the beds, is major problem in present time where the demand is increased, the ambulatory surgery unit has fixed amount of resources and can server special cases (minor surgery) more efficient and effective. Describe the factors that impact the establishment of an ambulatory surgery center. Two main factors should be considered in order to establish a successful ambulatory surgery unit: It must provide operative service of high standards of quality and safety at least equal to those of inpatient care. It must be both patient and cost efficient such that it provide high levels of patient satisfaction and is financially sustainable. [8] The ambulatory surgery centers usually formed from different facilities in the hospital, each facility has it unique characteristic and because of this uniqueness the ambulatory surgery units has to fulfilled these need. In general there are four categories of facility: Hospital integrated facility: in this type of facility the patients of ambulatory surgery are treated in partial or total through the inpatient units. Self-contained unit on hospital site: in such units the ambulatory surgery is performed in separate units, units build only for one day surgery and they are total separate from inpatient units. Freestanding self-contained units: those units look like the units which existed in the hospital. Physicians office-based unit: this is a limited resource units it can operate small surgery. It is important to identify these challenges and trying to control them to keep the quality and performance level high in the ambulatory surgery centers. Patient satisfaction is one of these difficulties that challenging the hospital to implementing the ambulatory surgery, it is hard to measure this amount of satisfaction and there is no certain baseline for this measure, but in general the patient is satisfied as long as that process goes as his/her expectation, which vary from patient to another according to their background and educations [24]. The productivity in the ambulatory surgery units can be measured using five types indicators [26]: Labor: because the nature of work in health care is based on labor, then the outcomes commonly represent service, with labor as input to the system the outcome could be clinic visits. All of the participants in health care (Patient, physician, and employee) should be prepared at the same time and processing the health care for same period of time, that is hard part to ensure the that load is distributed along the day, for some cases the maximum load at the morning and noon, but at midnight the load is less. Some engineering and system engineering tools and techniques are useful to solve this problem [26]. The number of outpatient and the patient in ambulatory is growing rapidly; the managers should manage the growing without direct them to another hospital or clinic is important strategy need very good management skills. That is one of difficulties that facing the ambulatory surgery centers. The challenging can be summarized as follow: Scheduling which can be happened because the numbers of patient is growing so fast and the need for good scheduling. Motivation: the employee performance is reducing while the number of patient is increasing and in the nearly future the needs for new strategy to motivate the employee. Patient satisfaction: with the number of patient increasing the number of different type of surgery in ambulatory surgery will increase. The need to standardized the procedure is growing. (f).Briefly illustrate the role of Industrial and Systems Engineering or Operations Management tools such as total quality management, process mapping, scheduling, etc., in establishing an effective and efficient ambulatory surgery center using at least two case studies or applications based on the literature. In the ambulatory surgery center the quality is essential rule to keep the performance and outcomes at acceptable level, also customer satisfaction is important in way to provide him/her with the wide range of treatment they required. At this step a continuous improvement and evaluation tools are the key element for ambulatory surgery centers to survive. A study suggested having a clinical indicators and acceptable thresholds that are relevant to daily practice, that will lead to easy measured and analyze the process parameters [25]. Modeling and simulation tool is good in way to provide the managers and the leader of ambulatory surgery units with virtual units where they can check the effect of increasing the demand on this type of surgery, where the most of hospital are looking to move their minors surgery to be done in separate units and leave the operation theater for the cases which need high attention and care. Enterprise-management tools, other useful tools to manage the ambulatory surgery units the idea is that units need to be managed as subsystem of the biggest system (Hospital). The interaction between hospital and ambulatory surgery units can provide these units and hospital with large amount of information of different cases and symptom that comes daily to the ambulatory surgery units and emergency department. In general industrial and system engineering tools can help the ambulatory surgery centers in different way, decrease the cost by decreasing the length of stay for patient: because most of the cases in the day surgery unit is normal and repeatable surgery (most of them are easy not complicated surgery) then the procedure can be standardized, this standardization will reduce the cost by removing the unnecessary process from the units. It is know that the nature of hospital is not same and there is high level of variation between different cases, this problem can be reduced in th e ambulatory surgery centers. Any engineering and system tools or techniques can be very useful in ambulatory surgery centers, in mean of the capabilities of providing the patient with excellent treatment within short period of time, both will result to patient satisfaction and improving the patient recovery time after leaving the ambulatory surgery centers, and also reduce the cost on ambulatory surgery center by reducing the number of employee and resources needed, because the good engineering tools mean less waste and more efficiently for both employee and resources. It can be summarized into following points: Decreased salaries expense by reducing the staffing requirements. Greater patient satisfaction through reduced waiting times. Increases employee satisfaction because workload is evenly distributed and activity levels do not create a hurry up and wait environment. Increases physician satisfaction because operations are more efficient and patients are more satisfied.

Even if you’re still using Exchange v5.5, Microsoft has you covered with a wealth of documentation to peruse. But what if you’re an Exchange 2000 organization that wants to bring in a new Exchange 2003 system alongside your existing machine, move all your content over to it, and decommission the original box? Then you’re left scratching your head. At the time of this writing, there is no guide I’ve been able to find that explains the process with any detail. This document will explain the process, combining information from numerous sources as well as my own experience. It’s very easy to bring Exchange Server 2003 into your Exchange 2000 organization, with minimal disruption to your existing server or your users. This document assumes you have an Exchange 2000 organization running in native mode. Henceforth, the Exchange 2000 system will be referred to as the â€Å"old† server, and the Exchange 2003 system will be referred to as the â€Å"new† server. I. Prepare your Network for Windows Server 2003 Regardless of how you intend to get to Exchange 2003, there are some basic steps that must be done. 1.  Ã‚  Ã‚  Ã‚  Ã‚  Begin by reviewing Microsoft’s 314649 – â€Å"Windows Server 2003 adprep /forestprep Command Causes Mangled Attributes in Windows 2000 Forests That Contain Exchange 2000 Servers† This article explains that if you have Exchange 2000 installed in your organization, and you proceed with installing your first Windows Server 2003 system (and its accompanying schema modifications), you may end up with some mangled attributes in AD. Preventing this from happening is simple enough: a script called Inetorgpersonfix.ldf will do the trick. 2.  Ã‚  Ã‚  Ã‚  Ã‚  Run adprep /forestprep from Windows Server 2003 CD on your Windows 2000 server that holds the Schema master FSMO role. (Of course you’ll need to be a member of Schema Admins). Be sure to replicate the changes throughout the forest before proceeding. 3.  Ã‚  Ã‚  Ã‚  Ã‚  Run adprep /domainprep from Windows Server 2003 CD on your Windows 2000 server. I ran it on the system holding the PDC Emulator FSMO role. 4.  Ã‚  Ã‚  Ã‚  Ã‚  Before bringing a new Windows Server 2003 system online, it’s a good idea to review your third-party server utilities and upgrade them to the latest versions to ensure compatibility.
